<p>The first thing you need to do is create a Google Spreadsheet.</p>
<p>Go to Google Docs, clickNew, then Spreadsheet.</p>

<p>Once you have a blank spreadsheet up, click Share on the right hand side.</p>

<p>Under the Share this document section there&#8217;s a new option labeled as &#8220;to fill out a form&#8221;.</p>

<p>Now your ready to edit the form.  Click Start editing your form&#8230;</p>

<p>Now you&#8217;re ready to modify your form.  Just like any other form you have the option to add text fields, paragraph fields, multiple choice, checkboxes, or choose from a list.</p>

<p>Now you&#8217;re done!  The form is automatically linked to the spreadsheet that you created earlier.  You can start inviting people to fill out the form or post the link directly to the form itself.  Here&#8217;s <a href="https://spreadsheets.google.com/viewform?key=p5zzH3CuSlTp6FDN_redqNw">the link to my example</a>.</p>

<p>Once people start filling out the form, the information is automatically populated in the spreadsheet.  You can then begin to work with this data.</p>
